Team captains are encouraged to review the Official Eligibility Rules prior to submitting their team registration.
Registration deadline is July 27, 2009.
Team registration for the ADA Military Cup Challenge MUST be done via the secure online form. Entry fees may be submitted online with Visa or MasterCard, or by mailing a money order directly to ADA Headquarters.
Team Captains make certain to gather all the following information from each of your team members prior to proceeding to the online registration form:
- ADA Membership number (if currently a member – new members can enroll online)
- First and Last name
- Current mailing address
- Social Security Number
- Singles event participation (Pre-registration only. After team registration and approval, no singles registration is allowed)
- Non-team event format (Neutralizer or Open for ALL their singles, doubles, mixed doubles and mixed triples events)
- Lowest PPDA (the lowest PPDA they should be allowed to compete at to maintain fair competition)
IMPORTANT: Teams that will submit their entry fees by money order, MUST print their confirmation email and mail along with the money order to ADA Headquarters:
